Admission Process

A phone call to Hospice of Dayton made by a patient, family member, friend, physician, or other healthcare professional begins the admission process. Our Admissions Staff will request general information and permission to contact the patient's physician in order to determine if the patient is eligible for hospice services. They will also offer to schedule a visit with the patient and family to explain hospice services.
