Clinical Teams
Hospice care involves nurses, physicians, social workers, chaplains, home health aides, volunteers and other healthcare providers in meeting the needs of patients and families. We provide care in a variety of service settings including:
Coming Home – a program designed to enable patients to remain at home, in familiar surroundings, while receiving care to manage physical, psychological and emotional concerns throughout their illness. Our Coming Home team has direct contact with the primary physician to provide comprehensive coordinated services.
Care Partners – services in partnership with extended care and assisted living facilities providing specialized care and support for physical, psychological and emotional concerns of patients. Working closely with each facility staff, Care Partners team members provide a wide range of services that enhance the patient's quality of life.
Comfort Care Centers – located in Dayton and Franklin, Ohio enable us to provide an option when care cannot be managed in the home or long-term care facility. Our Comfort Care in-patient facilities provide short-term services for pain and symptom management.
Career Opportunities
Nursing
Registered Nurses, Licensed Practical Nurses, Home Health Aides and Advance Nurse Practitioners all play vital roles in the nursing care provided to our patients.
Allied Health
Respiratory therapists, dieticians and other health related professionals help address individual patient needs to enable patients to remain in their home environment.
Chaplains
In keeping with the holistic philosophy of hospice care, chaplains address the spiritual needs of patients and families to help assure self-determined life closure.
Social Workers
As members of the interdisciplinary team, social workers assure that the needs of patients and families are addressed.
Pharmacy
Our pharmacy plays a pivotal role in addressing the medication needs of patients to assure pain and symptom management.
Palliative Care
Palliative care services are available to any patient with a chronic, progressive disease but who are not yet hospice eligible. Our Palliative Care clinicians help Manage pain and symptoms, establish care goals and assist patients and families making difficult treatment decisions.
Pathways of Hope
Pathways of Hope offers grief counseling for families and loved ones. Staffed by social workers and licensed grief counselors, our program provides one-on-one counseling and support group sessions to help cope with the loss of someone regardless of whether the loved one received care from one of our programs.
Administrative/Support Services
It takes a strong group of supporting professional players to support our clinical services. Human Resources, community outreach and education, development, volunteer services, clerical support, finance, accounting, information technology and marketing are all part of the business and administrative support functions. Facility support includes dietary, housekeeping and maintenance staff.
